Introduction

For Jammers:

We may collect personal information and / or health information about you such as:

1. your name, contact details, date of birth, gender, and ethnic identity;
2. your spoken language and preference for an interpreter;
3. your emergency contact details;
4. information about your mental and physical health (including medical history);
5. information about the support we have provided to you; and
6. details about your family.

For families of Jammers:

If you are a family member of a Jammer we are supporting, we may collect personal information about you such as:

1. your name, contact details, date of birth, gender, and ethnic identity; and
2. your spoken language and preference for an interpreter.

For volunteers:

If you are a volunteer, we may collect personal information such as:

1. your name, contact details, date of birth and demographic information;
2. your emergency contact details;
3. information about your volunteer role and commencement date; and
4. information about your suitability to provide services for StarJam.

For donors:

If you are a donor, we may collect personal information such as:

1. your name, contact details, date of birth; and
2. if applicable, your payment details.

How we collect information from you

We may collect personal information and / or health information about you:

1. through you contacting us directly;
2. through providing services to you (where relevant);
3. from other sources, including from third parties authorised by you such as health practitioners; and
4. through you making a donation on our website, including a 'Contact Us' form.

If you provide us with personal information and / or health information about another person, you should obtain their consent to do so, having informed them of this privacy policy where appropriate.

How we use this information

Any information that we collect from you will only be used for the purpose for which it was collected. Generally, this will be one or more of the following:

1. in relation to any correspondence you have entered into with us;
2. to process any applications, requests or donations you make to us;
3. to provide any services to you (where relevant);
4. for our own internal business purposes, including record keeping, reporting and improving/enhancing our services, activities and systems;
